Visiting one of our facilities for the first time?

To help expedite your check-in experience, please complete the following steps below to download, fill-in, and submit your completed registration forms to us. Also, please send us a copy of your pet(s)’ most recent medical records showing its vaccination history.

TIP: You may complete our forms electronically on your PC, Laptop, or Smartphone. To digitally sign your documents we recommend that you use Adobe Acrobat. If you experience any technical difficulties signing the forms, you may print off your completed forms and sign the hard copy with a traditional pen and then scan the document back in (a high-resolution photo from your smartphone works well too.)

 

STEP 1: Fill in a New Client Form and save it to your computer.

NOTE: If you have more than one pet, save a copy of the form for each pet and update any pet-unique information.

STEP 2: Download and complete the required Services Forms for the service(s) you are intending to use. Save the completed files to your computer.

STEP 3: Attach and email your completed forms, along with a copy of your pet(s)’ most recent medical records, to info@woofur.ca. In the subject line, please include your pet(s)’ first and last names e.g. “Registration forms for Teddy Smith”.
